Incorporating nurse navigation to improve cancer survivorship care plan delivery

This article evaluates the impact of incorporating a survivorship oncology nurse navigator (SONN) to improve survivorship care plan (SCP) delivery. Following the intervention, SCP completion increased, social determinants of health (SDOH) screening improved, and SCPs were translated for non-English speakers. Patients educated by the SONN had the highest SCP completion rate.
