This case report details the complete remission of metastatic gastrointestinal stromal tumor (GIST) in a pediatric patient, focusing on the successful therapeutic regimen involving high-dose imatinib induction for a rare, pediatric wild-type GIST that often lacks typical adult mutations. There was a favorable outcome for the patient who experienced multiple relapses since childhood. The need for more data and specific guidelines for treating GISTs in children due to their distinct characteristics was noted.
